Order Number

Enter the order number with your entry. All players who registered via our website received a confirmation email with an order number. We need this order number to verify the prize winner is the one entering this form. If you are a player who received entry elsewhere, please contact us.

Payment Options

All payments will be either PayPal or Zelle, we encourage Zelle because Zelle has no fees, if you select PayPal you will have a fee. We will begin sending payments out Tuesday following the tournament. Please give us 2-3 business days from the prize form submission to allow us to complete your transaction. After 3 months of the event end date, if a player has not submitted the prize claim form, the prize is deducted by 50%, and the prize is completely nulled after 6 months.

PayPal

PayPal has a fee (generally around 3%) so it is not the most efficient way to receive your prize, but we can do it. If you select this option, the player receiving the payment incurs the fee, not Universal, so please keep this in mind when selecting this option.

2. Zelle

Zelle has no fees associated with it, making it the preferred method. However, we want you to experience caution when giving us your Zelle email / phone number. If your info is incorrect, Zelle does not offer a way for us to dispute the transaction and re-route it.
